Getting Into Treatment
Accessing treatment starts with a chemical health assessment, which can be provided by Evergreen Recovery.
Do you need an assessment, or know someone who does? Call our office at (651) 252-6070 for more details and to schedule an appointment.
Have you recently had an assessment? Ask the provider who completed your assessment to fax it to us at (651) 252-6071, or email it to our Director of Admissions, Allison Canty, at allison@egrecovery.com.

Funding source
After we review the referral, we will contact you with our determination and/or to schedule an admissions date. Please note that depending on the funding source, you may be required to submit appropriate paperwork to the client’s county of financial responsibility.

Evergreen Recovery accepts various funding sources.
We currently accept the following insurance plans:

We also will work with clients who are on medical assistance, or have no insurance. Often, through the direct access process, we can provide an assessment and same day admit.
We accept self pay clients & commercial group plans.
No insurance? Not a problem. You’ll meet with one of our team members to complete an application for funding for your treatment through the Minnesota Behavioral Health Fund. Click the BHF Application button below to print & complete this form prior to arriving at our office. Please note that qualification for BHF funding is dependent upon income & household size.
